AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, LAIZA SOBERANO and FRANCISCO DOMINGO, both of legal age,

Filipino citizens, and presently residing at ________________________, after
being sworn to in accordance with law, hereby, depose and state that:

1. We know personally JORDAN MARQUEZ who was born on July 16, 1972
at Julio St. Cabuyao, Laguna. His parents are the spouses Roveliza
Marquez and .Raymart Marquez;

2. His birth was duly registered in the Office of the Municipal Civil Registrar of
Cabuyao, Laguna. His registered name is JORDAN MAGMOYAO
MARQUEZ.

3. In the GSSLAI record of said Jordan M Marquez his middle name was
inadvertently indicated as “MAGYOYAO” instead of “MAGMOYAO” which
is his true and correct middle name as registered in his certificate of live
birth.

4. That ever since he entered school and in all his other records, papers and
documents said Jordan M. Marquez has been consistently using
“MAGMOYAO” as his middle name.

5. We hereby declare and attest that Jordan Magmoyao Marquez and Jordan
Magyoyao Marquez refer to one and the same person.
