We hope you are doing well. We are writing this letter with much hope that you might
be able to help us. Our father/grandfather, ALFREDO V. BALTAZAR, widower, 75 years
old and a resident of Tupax, Jones, Isabela, has been admitted to the Intensive Care
Unit (ICU) of De Vera Medical Center, Inc, Santiago City, Isabela since October 10,
2020.

The Doctor’s diagnosis is Autoimmune Hemolytic Anemia and advised that they will
have to keep him at the hospital under observation for at least a week or over.

The issue is that at the moment, we are not financially stable and cannot afford the
hospital bill which is beyond our affordability. We are worried about the expenses we
would have to bear due to his health.
