JOINT AFFIDAVIT
We, ERLINDA ABINALES and JUSTINIANO ALEGRE both of legal age, both Filipino citizens, with residence address at Barangay Poblacion 3, Mendez, Cavite, after having been sworn to in accordance with law, hereby depose and state as follows: 1. That, We are bonafide residents of Barangay Poblacion 3, Mendez, Cavite, having born and lived here for quite a long time up to the present; 2. That, we personally know one JAIME H. VIDAMO, of legal age and presently residing at Barangay Poblacion 3, Mendez, Cavite; 3. That we know for a fact that JAIME H. VIDAMO was born on March 29, 1955 evidenced by the copy his Baptismal Certificate hereto attached and marked as Annex A; 4. That, ever since he came to reason he used the first name JAIME in all his personal records and other important documents up to the present time; 5. That, when he secured a copy of his Authenticated Certificate Birth Certificate from the National Statistics Office, his first name was entered and recorded as ARTURO; 6. That, JAIME H. VIDAMO is his true and correct name and not ARTURO VIDAMO as it appeared in his Authenticated Birth Certificate issued by the National Statistics Office (NSO); 7. That our knowledge concerning the above facts was based on our being close family ties with them; 8. That we execute this affidavit to attest the truth of what we have stated above and to inform all persons and offices concerned that his true and correct first name is JAIME and not ARTURO and that JAIME H. VIDAMO as appearing in his Baptismal Certificate and ARTURO VIDAMO entered and recorded in his Authenticated Birth Certificate is one and the same person; AFFIANT FURTHER SAYETH NAUGHT.
