JOINT-AFFIDAVIT OF TWO (2)

DISINTERESTED PERSONS
WE, EDUARDO R. TAMAYO and EUNICE A. GENTALAN, both of legal age, Filipinos,
married and residents of San Isidro, Brgy. Balagunan, Sto. Tomas, Davao del Norte,
Philippines, after having been sworn to in accordance with law, do hereby depose and state
that:

1. We are the same affiants who personally and voluntarily caused the preparation
and execution of this joint-affidavit;

2. We personally know the late EVELYN JARANTA MANLIGOY-VERGARA, who died

on September 15, 2013 at Davao Regional Medical Center, Brgy. Apokon, Tagum
City, Davao del Norte, Philippines;

3. Upon verification of her Death Certificate, it appears that the first name of her late
father, in the person of Pacito Lumantay Manligoy, was erroneously recorded as
JUANITO, instead of PACITO, which is the true and correct first name of her late
father;

4. In view of the foregoing, we hereby declare that the true, correct and complete
name of the father of the aforementioned person is PACITO LUMANTAY
MANLIGOY and that the names JUANITO LUMANTAY MANLIGOY and PACITO
LUMANTAY MANLIGOY, refers to one and the same person;

5. We are executing this joint-affidavit to attest to the veracity of the foregoing

declarations, to apprise the authorities concerned and for whatever legal purpose
this may serve best.
