JOINT AFFIDAVIT OF COHABITATION

WE, JEAN JESUS MARIDEO and JOE MARI BARRACA

APOLONIO, both of legal ages, Filipino citizens, single (living
together) and residents of No. 62 Anabu 1-A, Imus City,
Cavite, after having been duly sworn to in accordance with
law, do hereby depose and state THAT:

1. We are the above-named affiants as well as the

personal circumstances to whom they pertain;

2. We have been living together as husband and wife

under the same roof, continuously and without any
interruption, since June 2012 (more than Nine (9)
years);

3. During our cohabitation and even until the present, no

legal impediment or obstacle between us to marry each
other;

4. We were blessed with two (2) children namely JHOEM

MADRIDEO APOLONIO, who was born on July 23,
2013; and JHON GABRIEL MADRIDEO APOLONIO,
who was born on October 20, 2015;

5. We are willingly and voluntarily executing this JOINT

AFFIDAVIT OF COHABITATION to attest to the truth
of the foregoing statements and for whatever legal
purposes this may serve best.

IN WITNESS WHEREOF, we have hereunto set our
