AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, NORA H. ABELLANIDA, of legal age, married, Filipino and resident of Barangay
10, Catanauan, Quezon Province and AUREA T. FRONDOZA, of legal age, married, Filipino
and also a resident of Barangay 10, Catanauan, Quezon Province, after having been sworn to
in accordance with law do hereby depose and state that:

1. We personally know the deceased EDMUNDO C. SALUDES and CORA R. DANAO for we
are their long-time neighbor at Barangay 10, Catanauan, Quezon Province;

2. We are not related to them either by consanguinity or affinity and has no interest in
whatever this affidavit could be of use either directly or indirectly;

3. Since we have known the deceased EDMUNDO C. SALUDES and CORA R. DANAO for a
long time, our personal knowledge and circumstances brought the facts below:

a. We know for a fact that the deceased EDMUNDO C. SALUDES and CORA R. DANAO
cohabited with each other from August 2007 to April 2012;

b. That out of their cohabitation, a child was born: Carl Elvin D. Saludes on 12 March
2009; and

c. That when the deceased EDMUNDO C. SALUDES and CORA R. DANAO separated in
fact on 12 March 2009, and upon the death of Edmundo Saludes on 22 January
2021, Cora Danao was left alone to bear the sole parental care, custody, and
support of Carl Elvin D. Saludes; and

d. That Cora Danao became the guardian of Carl Elvin Saludes since the separation of
Edmundo Saludes and Cora Danao up to this date.

4. We are executing this affidavit to attest to the truth of the foregoing facts and for all legal
intents and purposes this may serve;

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 3rd day of
January 2023 at Catanauan, Quezon, Philippines.
