JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, LARRY BARTOLOME and WILSON SANTOS, both of legal ages, Filipinos,
married and with residence and postal addresses at Brgy. San Juan Concepcion,
Tarlac and Brgy. Tinang Concepcion, Tarlac, respectively, after having been duly sworn
to in accordance with law hereby depose and state that:

1. We personally know MARICAR BASCO of Brgy.Sto Nino, Concepcion, Tarlac,

being acquaintance for a long period of time now;
2. We declare, confirm, and affirm that she was born to Spouses Ricardo and Maria
Basco on May 24, 1999 at Tarlac Provincial Hospital as MARICAR and not
MARIACAR as appearing in her Certificate of Live Birth hereto attached as
ANNEX A;
3. She has been using the name “MARICAR” in all her legal documents and
transactions;
4. We are not related by consanguinity or affinity to the abovementioned person;
5. We are executing this affidavit to confirm and affirm all the foregoing facts and for
whatever legal purpose it may.

IN WITNESS WHEREOF, WE have hereunto set our hands this 10 th day of June
