AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, BIMBO ARAGON and LETICIA BALLO CARREON, both Filipinos, of legal
ages, and residents of Poblacion Sur and Bautista, Caba, La Union, respectively, having been
duly sworn to in accordance with law, hereby depose and state:

1. That we both personally know the person of AURORA ERUNDINA

CASTILLEJOS CARIŃO, over a long period of time because we live in the same
locality, and her family is a close family friend for several years;

2. That we know for a fact that the said person by the name of AURORA
CASTILLEJOS CARIŃO as known by many and as appearing in her other public
and commercial records with Cocolife Ins and AURORA ERUNDINA
CASTILLEJOS CARIŃO as appearing in her authenticated birth and death
certificate issued by the Office of Philippine Statistic Authority, refer to one and the
same person

3. That we are not related in any way, either be consanguinity or affinity, to the said
AURORA ERUNDINA CASTILLEJOS CARIŃO;

4. As such, we execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 19 th day of November,
