JOINT AFFIDAVIT

and residents of Tacloban City, having been duly sworn in accordance with law, hereby
depose and state:

1 That we have personally known the person of VICTORIA A. POLISTICO-

PASAYLOON for a long period of time having been her long time friend;

2 That we know for a fact that the person by the name of "VICTORIA" appearing
in some documents and records or specifically in her Certificate of Live Birth and
the name "MYRA VICTORIA " as appearing in her Marriage Certificate, refer to
one and the same person and that her true and correct name is” VICTORIA”;

3 As such, we execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve.
