JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, LEONIDA T. BUAYA, and MERLYN A. PUEBLA, of legal ages, both Filipinos
and residents of Carmen, Bohol, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That we personally know the person of BONIFACIA TESIO CASTRODES, for we are
friends thereof;

2. That as per BONIFACIA TESIO CASTRODES’s, Senior Citizen ID issued by the

Office of the Senior Citizens Affairs, Carmen, Bohol, her true and correct name is spelled
as BONIFACIA TESIO CASTRODES;

3. That as per BONIFACIA TESIO CASTRODES’s Certificate of Baptism issued by the

St. Anthony the Abbot Parish, Diocese of Talibon, Carmen, Bohol, her name was
erroneously spelled as BONEFACIA LABADAN TECIO, due to mistake and
inadvertence;

4. That the name BONIFACIA TESIO CASTRODES, as per her Senior Citizen ID issued
by the Office of the Senior Citizens Affairs, Carmen, Bohol, and BONEFACIA
LABADAN TECIO as per her Certificate of Baptism issued by the St. Anthony the
Abbot Parish, Diocese of Talibon, Carmen, Bohol, refers to one and the same person
living and existing in our community;

5. That we are not related to the said person either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
