I, ELIAZAR U. MOLINO, Filipino, of legal age, married, and a resident of Sitio

Fatima, Buenavista, Carmen, Bohol, Philippines, after having been duly sworn to in accordance
with law, do hereby depose and say:

1. That my correct and true name is spelled as ELIAZAR U. MOLINO;

2. That as per my Voter’s Certification issued by COMELEC at Carmen, Bohol, my true

and correct name is spelled as ELIAZAR U. MOLINO;

3. That however, under my Certificate of Eligibility issued by the Civil Service

Commission, my name was wrongly spelled as ELEAZAR U. MOLINO, due to mistake
and inadvertence;

4. That my name, ELIAZAR U. MOLINO under my Voter’s Certification issued by the

COMELEC of Carmen, Bohol, and ELEAZAR U. MOLINO under my Certificate of
Eligibility issued by the Civil Service Commission, refers to one and the same person
living and existing in our community;

5. That I execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
