JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MARGIE C. LAWIS, and MERLYN A. PUEBLA, of legal ages, both Filipinos
and residents of Carmen, Bohol, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That we personally know the person of JOSELYN CANINO, who is an illegitimate child
of Rosalina Canino who was formerly a resident of Calatrava, Carmen, Bohol;

2. That we know also for a fact that said JOSELYN CANINO was born on May 12, 1897 at
Calatrava, Carmen, Bohol;

3. That we know the fact of birth of JOSELYN CANINO on May 12, 1987 because we are
close family friend of Canino family;

4. That however, the fact of birth of JOSELYN CANINO was not recorded at the Office of
the Local Civil Registrar of Carmen, Bohol;

5. That we execute this joint affidavit to attest to the fact of the foregoing averments and to
support the application for a delayed registration of birth of JOSELYN CANINO before
the Office of the Local Civil Registrar of Carmen, Bohol;

6. The we further execute this affidavit for all intents and purposes this may serve.

IN WITNESS WHEREOF, we hereunto set our hands this 31 st day of January, 2023 at
