JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, SABAS L. REYES, and CELESTINO T. CEPADA, of legal ages, both Filipinos
and residents of Carmen, Bohol, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That we personally know the person of JOSELITO SARSABA VISTAL, for we are
friends thereof;

2. That as per JOSELITO SARSABA VISTAL’s Driver’s License issued by the Land
Transportation Office (LTO), his true and correct name is spelled as JOSELITO
SARSABA VISTAL;

3. That as per JOSELITO SARSABA VISTAL’s receipt issued by the Department of

Labor and Employment (DOLE) through its Tulong Panghanapbuhay Sa Ating
Displaced/Disadvantage Woker’s (TUPAD) Program, his name was erroneously spelled
as JOSELITO MARRIED VISTAL, due to mistake and inadvertence;

4. That the name JOSELITO SARSABA VISTAL’s as per his Driver’s License issued by
the Land Transportation Office (LTO), and JOSELITO MARRIED VISTAL as per his
receipt issued by the Department of Labor and Employment (DOLE) through its Tulong
Panghanapbuhay Sa Ating Displaced/Disadvantage Woker’s (TUPAD) Program, refers
to one and the same person living and existing in our community;

5. That we are not related to the said person either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.

IN WITNESS WHEREOF, we hereunto set our hands this 3rd day of March, 2023 at
Carmen, Bohol, Philippines.
