Dear Ma’am,

I write to you regarding your rentals for the property located at Gana,
Caba, La Union.

Under the lease agreement, your monthly rent of ____________________

(P_______) was supposed to be paid every ________________. However, your last
rental payment was made on October, 2020. As of this date, you have missed
four (4) monthly rental payments from November, 2020 to February, 2021
amounting to _____________________ (P__________). Due to your failure to pay
rent, you have also accrued a penalty of ________________ (P__________). In total,
you owe ___________ (P___________).

Despite of several demands your company failed to pay your overdue

rentals up to this present.

Due to the foregoing, I hereby make a final demand upon you to pay
_______________ (P_________) within five (5) days from receipt of this letter
otherwise I would be constrained to TERMINATE the lease agreement and to
file the necessary legal action (s) against your company to collect the foregoing
amount, plus 25% attorney’s fees thereon as well as litigation expenses.
