DEMAND FOR PAYMENT

Dear ______________________,

We write you upon the behest of our client, ____________________ of _____________________,

regarding the money you borrowed from her amounting to ____________________
(P______________) excluding the agreed interest of 20% per day.

Your obligation became due and demandable since April 2019, yet our records show that you have
note settled the full amount of _________________________ (P_____________________) excluding
the agreed interest of 20% per day. My client has approached you several times and demanded the
complete satisfaction of your obligation, regrettably, you refused to comply.

Thus, formal demand is being made upon you to pay the amount of _________________________
(P_____________________) excluding the agreed interest of 20% per day within ten (10) days from
receipt hereof.

You must realize that a court action is both tiresome and expensive. Coupled with this is the
embarrassment of having your name shared with CMAP (Credit Management Association of the
Philippines) and the Bankers Association of the Philippines which may negatively affect your loan
applications and credit standing with its member-banks/financial institutions. Should you fail to
comply with this demand, we assure you that we will proceed to prosecute your liability, be it civil or
criminal, to the full extent of the law.

Please consider this correspondence as our LAST AND FINAL DEMAND.

Very truly yours,
