AFFIDAVIT TO CANCEL ADVERSE CLAIM

I, MARK VINCENT N. MUÑOZ, of legal age, Filipino, married, a

resident of Jan Rey Subdivision, Tacloban City, after having been duly
sworn to, in accordance with law, hereby depose and state, THAT:

1. The undersigned is the affiant of an AFFIDAVIT OF ADVERSE

CLAIM dated 03 February 2021 which was executed before Notary
Public Atty. Kenneth Spice Medalla De Veyra, and annotated in TCT
No. T-2778 in the name of Marvin E. Nierras, under Entry No.
2022002967 inscribed on August 4, 2022, which read as follows:

“NOTICE OF ADVERSE CLAIM : EXECUTED BY MARK

VINCENT N. MUÑOZ, AFFIANT, CLAIMING AMONG
OTHERS THAT HE IS THE VENDEE AND THE LAWEUL
OWNER AND POSSESSOR OF TWO (2) PORTIONS OF A
PARCEL OF LAND COVERED BY TRANSFER
CERTIFICATE OF TITLE NO. T-2778 THROUGH A DEED
OF ABSOLUTE SALE EXECUTED ON JANUARY 25, 2019.
THAT HE HEREBY CLAIMS THE AFOREMENTIONED
PROPERTY AS HIS OWN, TO THE EXCLUSION OF ANY
THIRD PERSONS WHO HAS THE POSSESSION OF THE
OWNER'S COPY OF THE SAID TITLE, IN ACCORDANCE
WITH DOC. NO. 09; PAGE NO. 03; BOOK NO. X; SERIES
OF 2022; OF NOTARY PUBLIC OF TACLOBAN CITY;
ATTY. JEROME CORDERO AVILA; DATED JULY 29,
2022.”

2. I hereby request the Registry of Deeds for the City of Tacloban to

cancel or cause the cancellation of the annotation of the Affidavit of
Adverse Claim or Notice of Adverse Claim annotated on TCT No. T-
2778.

3. This affidavit is being executed to attest the truth of the foregoing

facts and for whatever legal purpose this may serve.

AFFIDAVIT OF LOSS

I, YIUKI MAE H. MUÑOZ, of legal age, married, and a resident of

Jan Rey Subdivision, Tacloban City, after having been duly sworn to, in
accordance with law, hereby depose and state, THAT:

1. I am the granddaughter of deceased Zenaida R. Buhay who died on

October 22, 2013 at Pasay City, Metro Manila;

2. The deceased Zenaida R. Buhay was a Social Security Member with

SS No. 03-0329973-8;

3. Upon application of her death benefits at the Social Security System,

one of the requirements thereto was the submission of her SS
Identification card and/or any valid ID;

4. I, together with my other family members, tried to locate the aforesaid

valid identification cards at Las Piñas City where she last resided but
to no avail. We also exerted diligent efforts to locate them in Tacloban
City but our efforts turned futile. Hence, we consider them legally
lost;

5. I am executing this affidavit to attest the truth of the foregoing facts

and for whatever legal purpose this may serve.
