JOINT AFFIDAVIT

We, JOSHUA FERNAN PALENCIA and DANILO DABAN, both of legal ages,
Filipino and residents of Tacloban City, after having been sworn to hereby depose and
state:

1. That I JOSHUA FERNAN PALENCIA is the registered owner of a motor

vehicle which is particularly described as follows:

2. That the original body type of said motor vehicle was Motorcycle without
Sidecar;

3. That I requested the services of DANILO DABAN to attach a sidecar in the

above-mentioned vehicle;

4. That recently, I DANILO DABAN, have attached a sidecar to said motor vehicle,
which attachment was not included in the registration;

5. That the change of body type is not intended for any illegal or unlawful purpose;

6. That we are executing this affidavit to attest to the truth of the foregoing and in
support for the application for the change of the Body Type of said motorcycle to
MOTORCYCLE WITH SIDECAR on its Certificate of Registration.
