AFFIDAVIT OF CHANGE BODY TYPE

We, REYNALDO S. MANLA, and, ORLANDO S. LAPA, Filipinos, of legal ages, and
a residents of Centro La Paz, Carmen, Bohol, Philippines, after being sworn to in accordance
with law, hereby depose and state:

1. That I, REYNALDO S. MANLA, am the registered owner of a certain motorized-

vehicle that is more particularly described as follows:

Type : MOTORCYCLE WITHOUT SIDE CAR

Chasis No. : KYA10121300
C.R. No. : 436555965
O.R. No.(2022) : 2185831970

2. That the original body type of the said motor vehicle is "Motorcycle without side car";

3. That to have more use of the said vehicle, I have decided to change the body type of the
said vehicle from " Motorcycle without side car " to "Motorcycle with side car";

4. That, I, ORLANDO S. LAPA, was the wielder who work with the above-mentioned
motorized vehicle, and attached and wielded a sidecar on it;

5. That we are executing this Affidavit to attest to the truth of the foregoing and in support
of the application for the change of body type of the above-described motor vehicle on
its Certificate of Registration.
