JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, EUSEBIA T. SERUT, and MAMERTA P. LABARRO, of legal ages, both

Filipinos and residents of Santa Cruz, Dagohoy, Bohol, Philippines, after having been duly
sworn to in accordance with law, do hereby depose and say:

1. That we personally know the person of the late MARCELINO P. FUERTES, for we are
friends thereof;

2. That as per MARCELINO P. FUERTES’ Certificate of Death issued by the Local Civil
Registrar (LCR) of Dagohoy, Bohol his true and correct name is spelled as
MARCELINO P. FUERTES;

3. That as per MARCELINO P. FUERTES’ Certificate of Group Life Insurance issued by

COCOLIFE, his name was incorrectly spelled as MARCELINO P. FUENTES, due to
inadvertence and mistake;

4. That the names MARCELINO P. FUERTES in his Certificate of Death issued by the
Local Civil Registrar (LCR) of Dagohoy, Bohol, and MARCELINO P. FUENTES as
shown in his Certificate of Group Life Insurance issued by the COCOLIFE, refers to one
and the same person living and existing in our community;

5. That we are not related to the said persons either by affinity or consanguinity and that we
have no interest in whatever outcome this presents may bring;

6. That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.

IN WITNESS WHEREOF, we hereunto set our hands this 10th day of March, 2022 at
Carmen, Bohol, Philippines.
