WE, MARICAR C. GALA and ARMANDO C. GARRIDO, both of legal age, Filipinos citizens,
married and both residents of San Pablo Brgy. Simangan and Brgy. Naungan Ormoc City,
respectively under oath, hereby depose and state:

1. I, MARICAR C. GALA, registered owner of a motorcycle without side car more with
Certificate of Registration No.44732347-2 particularly described as follows

from MOTORCYCLE WITHOUT SIDE CAR to TRICYCLE-FOR-HIRE;
3. That I, ARMANDO C. GARRIDO, was engaged to change the color of the above-described
motorcycle, which I personally did from MOTORCYCLE WITHOUT SIDE CAR to
TRICYCLE-FOR-HIRE;
4. That we executing this affidavit in order to attest to the truth of the foregoing and for the
purpose of compliance and requirements application for the change body of said motor
vehicle.
