I, EDWARD B. PEÑARANDA, of legal age, single, Filipino and with

residence at 59A Purok 9, Irisan, Baguio City, under oath, depose and say that:

1. I am the registered owner of a motorcycle (without Side Car) which

is particularly described as follows:

foregoing statements and to comply with the rules and regulations
of the Land Transportation Office anent the change of COLOR of a
motorcycle.
