SWORN STATEMENT ON CPC CONSOLIDATION

I, VITO GUMAPAC, Filipino, of legal age, married to Lolito M. Pasagad and a resident
of Purok 1, Sunrise Village, Tagbilaran City, Bohol, Philippines, after having been duly sworn to
in accordance with law, hereby depose and say:

1. That I am grantee of a CPC to operate an AIRCON TAXI under LTFRB Case No.
07-10-4760 on the line TAGBILARAN CITY AND TO ANY POINT IN BOHOL
PROVINCE, with the use of one unit, particularly described as follows;

2. That in compliance with the PUV modernization, I hereby already completed/and/or

processed and submitted my pertinent papers to Bohol Tourist Multi-Purpose
Cooperative as of April 11, 2019;

3. That I understood and voluntary affirm that my individual CPC and its authorized unit
should be consolidated and made part in a single juridical entity and that my individual
CPC will be voluntary dissolved to give way to its consolidation.

4. That I am aware that the failure to consolidate my CPC will result in the dissolution of
my CPC and the opening of the routes to applications filed by qualified juridical
entities such transport cooperatives and corporations.

5. That I attest the correctness of the foregoing statements;
