AFFIDAVIT OF LOSS

I, YVETTE MAUREEN DE DIOS OIDA, Filipino, of legal age, and a

resident of 1 E. Santos St, Concepcion Uno, Marikina City, after being duly
sworn in accordance with law, hereby depose and state that:

1. I am a holder of a Globe Sim Card No. 09550956688 & Union

Bank Debit Card issued by their designated office.

2. However, said Globe Sim Card and Union Bank Debit Card was
lost sometime on August 18, 2023 somewhere in the vicinity of
Bacolod, Negros Occidental.

3. Despite diligent search and efforts to locate said Globe Sim Card
and Union Bank Debit Card, the same could no longer be found
and is now beyond recovery.

4. As such, I am executing this affidavit to attest to the truth of the

foregoing and in support of my request/application for the
deactivation and replacement of the said Globe Sim Card &
Union Bank Debit Card.
