AFFIDAVIT OF LOSS

I, JUAN DEGAMO, of legal age, Filipino and a resident of 121

Yugyug Street Barangay San Isidro Labrador, Malolos, Bulacan, after
having duly sworn to in accordance with law, do hereby depose and state:

1. That I am the registered owner of a parcel of land which is

particularly identified as OCT No. CLOA-1234, situated in Yugyug
Street Barangay San Isidro Labrador, Malolos, Bulacan.
2. That I used to keep all my important documents including my
owner’s copy of the said title in a plastic folder and place it in one of
my drawers at my aforesaid residence.
3. That on the 28th of March 2022, my current residence was gutted
with fire and my drawer which contains all my important documents
were burned down including the copy of my title to the parcel of land
with OCT No. CLOA-1234.
4. That I exerted all my effort to save all my important files as well as
the copy of the said title. Unfortunately, the fire was uncontrollable.
5. That I did not sell, mortgage or in any manner dispose of my said
property to whosoever; and that there is no further transaction
involving said property for registration with said Register of Deeds/
6. That I am executing this affidavit to attest to all the above facts, and
to request the Honorable Register of Deeds of Bulacan to issue the
new owners duplicate of OCT No. CLOA-1234

AFFIDAVIT OF SELF-ADJUDICATION

I, CLAIRE A. JOAQUIN, of legal age, Filipino, single and a resident of 121

Yugyug Street, Barangay San Isidro Labrador, Malolos, Bulacan, after
having been duly sworn to in accordance with law, depose and state:

1. That I am the only daughter of Leonor A. Joaquin and Leon B.

Joaquin, who both died without any last will and testament on 28
March 2023 in Quezon City as evidenced by their respective Death
Certificate issued by the City of Quezon hereto attached as Annex
“A” and Annex “B” respectively, and made an integral part of this
Affidavit;
2. That the properties left by my parents are:
a. Insurance Policies issued by PhilAm Life Insurance Company
of USA (Philippines) amounting to one hundred thousand
pesos (Php 100,000.00), copy of insurance policies are
attached in Annex “C”, respectively, and made an integral part
of this affidavit;
b. Cash in Bank deposited in Bank of Isla de Pilipinas amounting
to one hundred fifty thousand pesos (Php 150,000.00), as
evidenced by a passbook attached in Annex “D”, and made an
integral part of this affidavit;
c. House and lot consisting of 80 square meters, valued at three
hundred fifty thousand pesos (Php 350,000.00), located in 202
Mark Street, Barangay Poblacion, Marilao, Bulacan,
Philippines more particularly described in TCT No. 38976,
attached in Annex “E” and made an integral part of this
affidavit;
3. That the properties left by my sibling are:
a. Jewelry worth seventy-five thousand pesos (Php 75,000.00)
found in her closet;
b. A wrecked vehicle, which was used by my parents and sibling
when the accident occurred, with a depreciated value of
twenty five thousand pesos (Php 25,000.00), as evidenced by
photographs, and Certificate of Ownership issued by the Land
Transportation Office attached in Annex “F” and “G”,
respectively and made an integral part of this affidavit;
4. That my sibling has a credit card obligation in the amount of sixty-
five thousand pesos (Php 65,000.00) in Banco de Pinas, as
evidence in the billing statement produced by the said bank attached
in Annex “I”.

 5. That pursuant to Rule 74, Section 1 of the Rules of Court, I hereby
adjudicate unto myself the above-described properties by means of
this Affidavit and hereby files the same with the Register of Deed of
Malolos City with the request that the said adjudication be made
effective without judicial proceeding as prescribed by the
aforementioned provision of the Rules of Court.

clearance;

charged under Criminal Case No. 466-1994-67 before the Regional
Trial Court, Branch 75 of Zamboanga City for the crime of three
counts of estafa under the Revised Penal Code;
3. That same person, Arnold P. Morgan, was also charged under
Criminal Case No. 466-1994-67 before the Regional Trial Court,
Branch 75 of Zamboanga City for the crime of Direct Assault
punishable under the Revised Penal Code;
4. That my True and Full name is ARNOLD PINTO MORGAN, and not
ARNOLD PARAS MORGAN.
5. That I am executing this affidavit to attest to the foregoing facts and
to deny categorically and specifically under oath that I am not the
same person mentioned in the docket, files, database of NBI with
said derogatory criminal records;

In view of the foregoing, I am requesting NBI Main Office in Taft

Avenue, Manila to issue the appropriate NBI clearance in my favor for the
purpose of applying another job in Dubai as a Site Engineer.

 RESOLVED as it is hereby resolved, that CLEMENTE SOLIS,
with the assistance of the Corporation’s counsel of record, is
authorized to:
a. Initiate, verify, certify, authenticate, file, execute, and/or cause
the filing of the complaint, position papers, reply, rejoinder,
appeal, memorandums, and/or petitions with the proper courts
or offices of the government and/or notary public for the
enforcement, satisfaction and/or defense of the rights of the
Corporation under such agreements contracts and/or
documents;
b. Sign and/or execute such documents, affidavits, verifications
and/or certifications required in the filing and/or defense of such
complaints and/or petitions as well as answers and other
pleadings;
c. Represent the Corporation in any litigation covering the
aforementioned agreements, contracts, and/or documents,
including negotiations and entering into arrangements to work
out an amicable settlement or compromise agreement in such
litigation, administrative proceedings, and pre-trial conferences,
to do and perform the following: (i) execute the certificate of
nonforum shopping in accordance with law and Supreme Court
Circulars; (ii) enter into an amicable settlement or submission to
arbitration and to agree to the same; (iii) submit alternative
modes of dispute resolution, if applicable; (iv) appoint or

whatever nature or kind as may now become due, owing
payable or belongs to the Corporation, and to have, sue, and
take any and all lawful ways and means for the recovery hereof
by suit, attachment, compromise or otherwise, in connection
therewith;
e. Make, sign, execute, and deliver contracts, documents,
agreements, and other writings of whatever nature or kind, with
any and all third person/s, concerns, or entities, upon terms and
conditions acceptable to the said attorney;
f. Delegate in whole or in part or all the powers granted or
conferred herein, by means of executing an instrument in
writing, in favor of any and all third person/s whom the
representative may choose; and

g. Sign and execute documents necessary to perform the acts
enumerated in the preceding paragraphs.

RESOLVED FURTHER, as it is hereby resolved, that the

foregoing authority shall continue to be in full force and effect
until revoked by the Board of Directors in a resolution duly
certified by the Corporate Secretary;
RESOLVED FINALLY, that the foregoing resolution has not
been amended, modified or repealed and that the same are still
in full force and effect”
