AFFIDAVIT of EXPLANATION
  
I, JUANA DE LA CRUZ, of legal age, Filipino, married, and a resident of OPOL,
MISAMIS ORIENTAL, after having been duly sworn to in accordance with law,
depose and state THAT:

1. I am the holder and owner of a Philippine Passport with the number

“P6123456A” issued by the Department of Foreign Affairs Cagayan de Oro
City on 20 February 2018 with an expiry date of 19 February 2028;

2. I got married in 2020. Sometime in August 2023 and prior to the expiration of
my said Passport, I erroneously applied for a new passport as I wanted to
change my name indicated therein to that of my new name as a married
person;

3. I had no knowledge that the proper procedure was to apply for a renewal of my
passport, instead of the application of a new passport;

4. Such erroneous act on my part was purely due to mistake and not in any way
made for fraudulent purposes;

5. I am voluntarily executing this affidavit to attest to the truth of the foregoing

information and to apply for the renewal of my passport with the Department of
Foreign Affairs Regional Consular Office, Cagayan de Oro City;
