2285 Chino Roces Ave Extension, Makati City, Rizal, after having been sworn in accordance
with hereby depose and say that:

1. I am the Account Consultant of Starfire Holdings Corp. and have been authorized by
plaintiff to represent, compromise, and institute the present case for and in their behalf,
as shown by the Secretary’s Certificate, a copy of which is attached hereto and made an
integral part hereof;
