AFFIDAVIT OF EXPLANATION

I, MARY LAURENCE MICHELLE A. LAGRIMAS, of legal age,

Filipino, and a resident of Davao City, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1. I was issued on 02 July 2019 a Philippine Passport with Passport no.

P2406843B, which is valid until 01 July 2029;

2. The said passport was damaged in April 2022 when I was cleaning my
backpack in my washing machine. Unfortunately, my passport was
inside my backpack at the time, thereby inadvertently causing
discoloration and wrinkles to it. The water also caused it’s cover to
dwindle, thereby losing its previous integrity;

3. The damage caused to my passport was inadvertent and unintentional;

4. I am attaching to my Affidavit of Explanation a copy of my Certificate

of Live Birth as proof of my ownership of the Philippine Passport No.
P2406843B;

5. I am executing this affidavit to attest to the truth of the foregoing, to

support my application for a new passport to replace the damaged one,
and for whatever purpose it may serve.
