PETITION

PETITIONER most respectfully files this application for

appointment as a Notary Public for and in the City of Lapu-Lapu, for a
term commencing on January 1, 2023 and ending on December 31, 2024
and avers that:
1. Petitioner possesses the eligibility for commissioning as Notary
Public as shown by the following personal circumstances:
a) Nationality : Filipino
b) Date of Birth : October 16, 1991
c) Place of Birth : Lilo-an, Cebu, Philippines
d) Age : 31 years old

2. Petitioner is currently residing in Lilo-an, Cebu. She is a resident
and a natural born citizen of the Philippines since birth.

3. Petitioner holds office at the following address:

4. Petitioner has satisfactorily completed the required course for the

Juris Doctor (JD) degree at the University of San Jose-Recoletos in
April 2020. She took and passed the 2020-2021 Bar Examinations
and was admitted into the practice of law on May 30, 2022 under
Roll of Attorney’s No. 82442. A copy of the Certificate of Bar
Membership is attached herein as Annex “A”.

5. Petitioner has neither been convicted nor charged with any

offense involving moral turpitude or any criminal or civil offenses
for that matter. A copy of the Clearance issued by the National
Bureau of Investigation is attached herein as Annex “B”.

6. Petitioner is a member of the Philippine Bar in good standing as

shown by the following certifications:
a. Certification of No Pending Administrative Case issued by
the Office of the Bar Confidant of the Supreme Court of the
Philippines which is attached herein as Annex “C”,

b. Certificate of Good Standing and of Good Moral Character

issued by two (2) executive officers of the Integrated Bar of
the Philippines (IBP) - Cebu Chapter, attached herein as
Annex “D”.
7. Petitioner is a registered member of IBP – Cebu Chapter and has
duly paid her IBP Membership dues for the current year as

 evidenced by Official Receipt (OR) Number 278379 dated January
9, 2023. A printout of the payment history section in Petitioner’s
MYIBP App Account showing such payment is attached herein as
Annex “E”. A copy of the Official Receipt will be submitted during
the summary hearing.

8. Petitioner is MCLE-Compliant, having obtained 36 credit units of

Mandatory Continuing Legal Education for the 8th Compliance
Period (April 15, 2022 to April 14, 2025), with MCLE Compliance
Number 0001318 valid until April 14, 2028. A printout of the
electronic copy of the Certificate of Compliance is attached herein
as Annex “F”, the original copy thereof will be presented during
the summary hearing for comparison.

9. In compliance with the requirements of the Honorable Court

for Notarial Commission, attached hereto are the following
annexes and made an integral part of this petition:

a) Professional Tax Receipt (PTR) for the year 2023 which

is attached herein as Annex “G”
b) Three (3) passport size colored photographs with
signature at the bottom, taken within thirty (30) days
from the date of filing of the instant Petition as Annex
“H”
c) Three (3) specimens of Petitioner’s signature as Annex
“I”

10. Petitioner now desires to be appointed and commissioned

as Notary Public acting for and in the City of Lapu-Lapu, for the
term ending on December 31, 2024.

 11. Petitioner has not applied for nor was she issued a
Commission as a Notary Public in any other jurisdiction for the
current year.

12. Petitioner has paid in full the proper filing fees.

13. Petitioner undertakes to submit or report to the Office of

the Clerk of Court copies of all documents and deeds notarized by
her.

14. Petitioner possesses all the qualifications and none of the

disqualifications and desires to be appointed as a Notary Public
for and in the City of Lapu-Lapu and within the jurisdiction of this
Honorable Court.

PRAYER

WHEREFORE, Petitioner most respectfully prays that she be

appointed and commissioned as Notary Public for and in the City of
Lapu-Lapu and within the territorial jurisdiction of this Honorable
Court. It is likewise prayed that Petitioner be authorized to purchase her
notarial seal for that purpose.

I, GLADYS S. CAÑETE, Filipino, of legal age, single and a resident

of Purok Sitaw 4, Bagacay, Catarman, Lilo-an, Cebu, after being duly
sworn to in accordance with law, hereby state that:
1. I am the petitioner in the above-entitled Petition;
2. I have caused the preparation of the said Petition;
3. I have read the foregoing Petition and I attest that the allegations
contained therein are true and correct based on my personal
knowledge and/or authentic documents.
4. The factual allegations therein have evidentiary support.
5. I hereby certify that I have not commenced any other action or
filed any claim involving the same issues in any court, tribunal or
quasi-judicial agency and, to the best of my knowledge, no such
other action or proceeding is pending therein.
6. If I should thereafter learn that a similar action or proceeding has
been filed or is pending before any court, tribunal or quasi-judicial
agency, I undertake to report the said fact within five (5) days
therefrom to this Honorable Court.
