COMES NOW, the petitioner in the above-entitled case, by himself,

and unto this Honorable Court, most respectfully alleges:

1. That he is a Filipino, 28 years old having been born on January 01,

1990, single and a resident of 0073-A C. Mandaue City, Cebu with
mobile no.(s) 123456;

2. That he passed the Bar Examinations given in the year 2016 and
has since then and up to the present a member in good standing of
the Integrated Bar of the Philippines and/or the Integrated Bar of
the Philippines, Bohol Chapter with Roll No. 12345, May 24,
2017;

3. That he has been a resident of the Philippines since birth and

maintains a regular place of work or office in the City of Mandaue
and the Province of Cebu for the last one (1) year and six (6)
months;

4. That he has been appointed and commissioned as Notary Public for

the City of Mandaue and the Province of Cebu starting 2017 until

 the present. The Notarial Commission was issued last July 26,
2017 and will expire on December 31, 2018;

5. That he desires that said commission be again renewed/extended

for another period of two (2) years up to December 31, 2020 and
that he possesses all the qualifications and none of the
disqualification for the said office pursuant to the Notarial Law;

6. That he has paid the necessary Professional Tax Receipt (PTR) as

a lawyer for the year 2019 under O.R. No. 12345 dated December
12, 2018 (For 2019) at Tagbilaran City, Bohol and has likewise
paid the IBP dues for the year 2018 as evidenced by A.R. No.
12345 and 12345 issued by the IBP Cebu Chapter, machine copies
of which are hereto attached as Annexes “A”, “B” and “B-1”
respectively, and made as integral part of this Petition;

7. That he is submitting the specimens of his signatures and

facsimiles of his notarial seal on a separate paper hereto attached
and marked as Annex “C” and Annex “D” respectively;

8. That he is likewise submitting a certification of good standing by

the two (2) officers of the local chapter of the Integrated Bar of the
Philippines in this place where he is applying for renewal of his
notarial commission and hereto enclosed and marked as Annex
“E” and Certificate of No Pending Case from the Supreme Court
hereto attached as Annex “E-1”;

9. That enclosed hereto are three (3) passport size color photographs
of herein petitioner with his signature appearing at the bottom part
of said photographs hereto marked as Annexes “F”, “F-1” and
“F-2”;

10. That furthermore he has not been convicted nor charged of any
crime involving moral turpitude before any court or tribunal in the

 Philippines and likewise submitting his NBI Clearance hereto
attached and marked as Annex “G”;

WHEREFORE, it is most respectfully prayed unto this Honorable

Court that his Commission as Notary Public for the Province of Cebu and in
the City of Mandaue be please GRANTED/EXTENDED/RENEWED.

Petitioner further prays for other reliefs and remedies just and
equitable under the circumstances.

2. That I have caused the preparation of the foregoing Petition;

3. That I have understood the entire contents hereof which are true
and correct to the best of my personal knowledge and based on
authentic records.

 4. That I have not theretofore, commenced any other action involving
the same issues in the Supreme Court, the Court of Appeals or
different division thereof, or any other tribunal or agency. That if
there is such other action or proceeding, I must state the status of
the same; and if I should thereafter learned that a similar action or
proceeding has been filed or is pending before the Supreme Court,
the Court of Appeals, or different divisions thereof, or any other
tribunal or agency, I will undertake to promptly inform the
aforesaid courts and other tribunal or agency thereof within five (5)
days therefrom.
