PETITION

Petitioner, unto this Honorable Court, most respectfully states

that:

1. He was commissioned by the Regional Trial Court in Davao

City as Notary Public with Commission Serial No. 0000-0000-0000.

2. His commission as Notary Public for and in the City of Davao

will expire on December 31, 2035.

3. He respectfully requests to renew his commission as Notary

Public for and in the City of Davao upon expiration thereof pursuant
Section 13, Rule III of A.M. No 02-8-13-SC dated July 6, 2004;

4. Attached hereto, as Annex “A”, are three specimens of his

signature and a true impression of his dry seal made pursuant to
law;

5. A photocopy of his Professional Tax Receipt No. 12345678,

issued at Davao City on Febuary 5, 2019 is hereto attached as
Annex “B” hereof;

6. A photocopy of his IBP Official Receipt No. 1111111, issued

at Pasig City on July 27, 2019 is hereto attached as Annex “C”
hereof;
7. A copy of his Certification of Membership in Good Standing
dated December 24, 2008 signed by two (2) executive officers,
Gerardo F. Del Rosario in his capacity as Director and Rosalina V.
Pascua in her capacity as Secretary, both of the Philippine
Securities and Exchange Commission attached as Annex “D”
hereof;

10. A copy of his NBI clearance per OCA Circular No. 5555-5555
is hereto attached as Annex “E” hereof;

11. He has all the qualifications and none of the disqualifications

for said office;

WHEREFORE, premises considered, it is most respectfully

prayed that Petitioner’s commission as Notary Public for and in the
City of Davao be renewed for the years 2023-2024.
