AFFIDAVIT OF SELF-ADJUDICATION

I, LEONORA JOAQUIN, of legal age, single, Filipino and residing/with

address at 1635 Yakal St., Green Fields, Molino 1, Bacoor, Cavite, having been
duly sworn in accordance with law, hereby depose and state:

1. THAT I am the only daughter of LEON JOAQUIN who died intestate on

April 12, 2021 in Bacoor City as evidenced by his Death Certificate issued
by the City of Bacoor;

2. THAT at the time his death, my father had no debts, liabilities or

obligations to any persons, agency or institution;

3. THAT said deceased person at the time of his death left the following
properties:

a) A parcel of land known as Lot No. 1006-A-3, particularly described

as follows:

A parcel of land known as Lot No. 1006-A-3, of the subdivision plan

(LRC) Psd-854788, situated in Molino 1, Bacoor City, Province of
Cavite containing an area of 150 sq. m., or less covered by
Transfer Certificate of Title No. T-8349123 of the Registry of Deeds
of Trece Martires, with an assessed value of P100,000.00;

b) A residential house built on a portion of Lot No. 1006-A-3, situated

at Bacoor City with an assessed value of P250,000.00;

c) A wrecked 2018 Toyota Altis with a depreciated value of

P25,000.00

d) Death proceeds attributable to his Policy No. LJ-654 issued by Sun

Life Financial valued at P100,000.00

4. THAT pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby

adjudicate unto myself the above-described real estate by means of this
Affidavit and hereby files the same with the Register of Deed of Trece
Martires with the request that the said adjudication be made effective
without judicial proceeding as prescribed by the aforementioned
provision of the Rules of Court.

         IN WITNESS WHEREOF, I have hereunto set my hand this 13th day
of April 2021 in Bacoor City, Cavite, Philippines.
