AFFIDAVIT OF LOSS
I, ROSSANA D. SADOGIO, of legal age, single, Filipino, and a resident of 1213A
Cardona St. Brgy. Poblacion., Makati, Philippines, after having been sworn to in accordance
with law hereby depose and state that:

1. I was in possession of a cellular phone with Sun SIM card with cellphone
number 09322730926;

2. We bought a local sim card in Vietnam on July 17, 2023, and the salesperson
there remove my Sun and Gomo SIM card off of my phone. I forgot to verify if
she gave it to me, and only recently did I realize that it was not in my purse.

3. I took diligent effort to locate my sim card but I could not find the same, hence,
I believe it is now beyond recovery.

4. I am executing this Affidavit of Loss to attest to the truth of the foregoing

statement, and to request for a replacement of my Sun SIM card, and for
whatever legal purpose it may serve.
