AFFIDAVIT OF LOSS

  I, AUBREY JOYCE BAUTISTA, of legal age, Filipino, single and residing

at 65 Tamarind St., South Forbes Park, Makati City, after having been duly to
sworn in accordance with law, do hereby depose and state that:

1. I am a holder of a School I.D with a student No. 2019-102332;

2. Sometime in Mandaluyong City in March 2023, the said School I.D was
lost on my way home. I put it in the pocket of my jacket, but I was unable
to locate it along with my beep card and vaccination card.

3. That the said School I.D is currently used as one of my proofs of being
enrolled.

4. Efforts were exerted to recover the said School I.D, but in spite of diligent
search, it could not be found and the same is now beyond recovery;

5. As such, I am executing this Affidavit of Loss to attest to the truth of the

foregoing and to support the issuance of a new one in lieu of the one
which was lost and for whatever legal purposes it may serve.
