depose and say that:

1. That I am a regular user of a Globe Mobile SIM Card with No.

09657424531;

2. That on January 19, 2023, The SIM CARD with a contact

number 09657424531 that I had been using for my GCash account
had been deactivated without my knowledge.;

3. That I hereby declare and respectfully request to transfer my

funds with the amount of twelve thousand Plus (P12,000+) on
my GCash account from my old SIM Card No. 09657424531 to
my new SIM Card No. 09665995587;

5. I am executing this affidavit to attest to the truth of the foregoing

facts and in support of my application for transferring my funds
and for whatever legal intents and purposes it may serve.
