AFFIDAVIT

KNOW ALL MEN BY THESE PRESENTS:

I, PILIPINO CAYETANO, Filipino, of legal age, and with postal

address c/o Coca-Cola Bottlers Philippines, Inc., 4th Floor, king’s Court
Building II, 2129 Pasong Tamo, Makati City after having sworn in
accordance with the law, hereby depose and state, That - -

1. I am the Production Manager of Coca-Cola Bottlers

Philippines at Sta. Rosa Plant from __________ to
_______________;

2. As a Production Manager, I was in-charge of overseeing

the production line consisting of four (4) bottling lines;

3. On 6 July 2009, at around 12:55 p.m., I noticed a certain

employee, Felicisimo Articona, conducting the inspection of
Coca-Cola products with his eyes closed;

4. At that time, I was at my office located at the Metal Deck

located above the bottling lines. The metal deck is
approximately five (5) meters high from the ground, high
enough to completely watch over the entire production
line;

5. The metal deck has a stair directly leading to one bottling

inspection line where Felicismo Articona was stationed at;

6. While I was watching over the production line and going

down the stairs, I noticed Felicisimo Articona with his eyes
totally closed;

7. When I reached his station and stood beside him, his eyes
were still closed. Approximately, he has been inspecting
goods with his eyes closed for almost two minutes. In fact,
his head is already swaying a bit;

8. He just opened his eyes after being alarmed by the

banging sound caused by the knocking over of the bottles
at the conveyor;

9. When he opened his eyes, he was surprised to see me

beside him and merely uttered “Pasensya na po”;

 10. After that incident, I called the attention of his immediate
supervisor, Armando Enriquez, via a text message stating
the careless inspection of Felicisimo Articona;

11. Thereafter, I formally sent an e-mail message to Armando

Enriquez to completely inform the latter of the incident and
directed him to issue the corresponding Notice to Explain
to Felicisimo Articona for careless inspection of products;

12. During break time of the employees, Felicisimo Articona

approached me and ask for forgiveness regarding the
incident where I caught him with his eyes closed while
doing the inspection of goods;

13. Thereafter, investigations were conducted against

Felicisimo Articona. During the investigation, I reviewed his
201 Personnel File and discovered that he has two
previous infractions for careless inspection of products.
Thus, it would appear that the above-stated incident is
already his third offense. I conveyed the foregoing matter
to Armando Enriquez for notation;

14. I am executing this affidavit to attest to the truth of the

foregoing statements for whatever legal purpose it may
serve.
