AFFIDAVIT OF WITNESS

I, MARK RONNEL L. TAVAS, of legal age, Filipino, and a resident of Consolacion,

Agoo, La Union, after having been duly sworn to in accordance with law, hereby depose and
say:

1. That I am a tricycle driver by occupation;

2. That on February 3, 2024 at around 3:30 o’clock in the afternoon, I was driving my
tricycle along Verceles St., Consolacion, Agoo, La Union when I paased by a “scene”
which caught my attention;

3. That I saw a male person being held by two (2) individuals;

4. That I recognized him as the lotto-bet collector by the name of ALEXANDER DE

GUZMAN;

5. That I likewise recognized the two (2) male persons who were holding Alexander and
they are police officers whom I sometimes see manning the police outpost at the Jose
D. Aspiras Civic Center;

6. That the place where Alexander was held by the police is near a police outpost
located at the PNB-Agoo and where police officers would conduct traffic
enforcement;

7. That Alexander even asked me to help him but I cannot do otherwise;

8. That I am executing this affidavit to attest to the truth of the foregoing facts and for
all legal intents and purposes.
