AFFIDAVIT OF LOSS

I, EILEEN ANUNCIACION GONZALES, of legal age, single, Filipino,

and residing at #240 Brgy., Sto Niño, Baliuag, Bulacan after having been duly
sworn to in accordance with law, hereby depose and state that:

1. I am a registered Real Estate Broker with Reigistration No.;

2. That I normally kept my Board Certificate and all my pertinent

documents in a brown envelope in my drawer cabinet;

3. However, last September 2009, Typhoon Ondoy came which

caused enormous floods in our municipality. Because of such, we
were forced to remove all our things, including the said brown
envelope where my documents are kept, from our house and
tried to move them to a safer place;

4. After the said incident, I tried to locate all my things and

documents, but to my dismay I can no longer find the said brown
envelope due to the commotion and confusion brought about by
the typhoon;

5. I exerted due diligence to locate said documents by looking and

searching with the other important documents in my possession,
but all my efforts proved futile, as I could no longer find the same.

6. For the aforesaid reasons, the same is now considered lost and
beyond recovery;

I am executing this Affidavit of Loss to attest the truth of the loss of said
Board Certificate and for whatever legal intents and purposes it may serve.
