AFFIDAVIT OF UNDERTAKING

I, TRICIA MAE M. PEÑA, of legal age, Filipino and with residence and postal
address at 169 Duhat Ext., Napico Manggahan, Pasig City, after having been duly
sworn to in accordance with law do hereby depose and state:

1. That I am graduating student of the Polytechnic University of the

Philippines with Student Number 2015-06902-MN-0;
2. That in compliance with the requirements of the aforementioned University,
I hereby undertake to submit a (a)Corrected/Annotated Certificate of Live
Birth issued by the Philippine Statistics Authority, (b) Affidavit of
Discrepancy, (c) Affidavit of Two Disinterested Persons;
3. That I fully understand that compliance of the abovementioned
requirements is strictly mandatory and failure to submit the same shall
prevent the University to release my school records;
4. That I likewise undertake and assume full and complete responsibility for all
the consequences which may arise due to the non-issuance of my school
records;
5. That I shall not hold the University liable for not releasing my school records
until I have fully complied with the requirements mentioned above; and
6. That I am executing this affidavit to in order for the University to proceed
with the evaluation of my scholastic records for my graduation pending
submission of the aforementioned requirements.
