AFFIDAVIT OF UNDERTAKING

I, MARK EDSON VEGA ABOY, of legal age, married, Filipino, with

residence and postal address at Upper Musi Musi, Blanco, Balingasag,
Misamis Oriental, after having been duly sworn to in accordance with
law, hereby depose and state that:

1. That I was a bona fide student of the College of Graduate

School, Medina College-Ozamis City from SY 2015-2016 2 nd
Semester to SY 2018-2019 and finished my Master’s Degree of
Elementary Education (MAEED);

2. That during the period of my graduate studies, I have attended

classes in Medina College campus located in Maningcol Ozamis
City;

3. That during the same period, I was connected with the

Department of Education, Division of Misamis Oriental;

4. That all school fees and other miscellaneous expenses incurred

for my graduate studies was paid on a personal capacity;

5. That this affidavit is being executed to attest the truth of the

foregoing and pursuant to and in compliance with the
requirements set forth by the Medina College for my request of
issuance of CHED SPECIAL ORDER.

IN WITNESS WHEREOF, I am affixing my signature this 1st day of
