SWORN AFFIDAVIT

I, _____________, 60 years of age, Filipino and a resident of Brgy. Malinao,

Banisilan, Cotabato, after having been sworn to in accordance with law, do hereby
depose and state:

1. That I am currently employed as Master Teacher II in DEP-ED and

assigned in Malinao Central Elementary School;

2. That I will be turning sixty-one (61) years old on September 9, 2020;

3. That I am still willing to work for now and still have no intention of
retiring soon;

4. That intend to retire when I reach sixty-three (63) years old or on the year
2022;

5. That I am executing this affidavit to attest to the truthfulness of the

foregoing and for whatever legal purpose this may serve best.
