JOINT AFFIDAVIT OF SUPPORT

WE, BEN R. PIANO and MARITES A. PIANO, both of legal ages, spouses, Filipinos,
married, respectively and both are residents of Brgy. San Francisco, Lagonoy, Camarines Sur,
after having been sworn in accordance with the law, hereby depose and state that:

1. We, are the legitimate parents of Emy Eloisa A. Piano, 20 years of age, single, Fourth
Year Pharmacy student of the University of Sto. Tomas, Manila, and an academic
scholar thereof;

2. Our daughter, Emy, is interested to be admitted as a Medicine student at the

Pamantasan ng Lungsod ng Maynila, College of Medicine this coming S/Y 2019-2020;

3. We have sufficient income/assets to responsibly cover our daughter’s expenses during

her entire admission as a Medicine student through my retirement pension from the
Philippine National Police;

4. As her parents, we hereby accept and assume any and all of financial expenses, as well
as any and other unforeseen expenses which our daughter may incur during her entire
course of study;

5. This affidavit is executed for the purpose of attesting to the truth of the foregoing facts, in
support of our daughter’s studies.
