AFFIDAVIT

OF

LOSS

I, CHERRYLYN VILLAVIRAY, Filipino, of legal age, single and

residing at 199 Calamba Ext., Brgy. Talayan, Quezon City, after having
been duly sworn to in accordance with law, do hereby depose and state:
1.

That I am a holder/possessor of a the following documents:

TRANSCRIPT OF RECORDS; AND

COLLEGE DIPLOMA

both issued by my alma matter, METROBUSINESS COLLEGE.

That the said afore-stated documents was safely placed on
my files and documents in our house situated at my
aforementioned residential address;
3.
That sometime on August 7, 2011, our place was heavily
flooded due to heavy rains (bagyong habagat) and unfortunately
because of the incident the abovementioned documents were lost
and nowhere to be found;
4.
That despite diligent and exhausted efforts exerted the same
could no longer be found, by reason of which I am declaring and
considering the said afore-stated documents as lost and beyond
recovery;
5.
That I have executed this instrument as a compliance with the
requirements needed for the issuance of a new copy of
TRANSCRIPT OF RECORDS and COLLEGE DIPLOMA at
METROBUSINESS COLLEGE;
6.
That I have executed this affidavit to attest to the truthfulness
and veracity of all foregoing fact and for whatever legal intent this
may serve, especially for the information of all the authorities
concerned.
IN WITNESS WHEREOF, I have hereunto set my hand and affixed
