AFFIDAVIT OF CONSENT
(TO THE ADOPTION)

I, JHASMINE RHIANE SELDA HERNANDEZ, Filipino Citizen, born on

September 23, 2009(10 years old), and with address at No. 19 Marcoville, Baguio
City, after having been duly sworn to in accordance with law, hereby depose and
state that:

1. I am the youngest(fourth) child of KARILAGAN SELDA;

2. My biological or my real father is JONATHAN ATIJERA OSTIL while my other

siblings are from another father;

3. I also have a younger sibling named Samantha Kate E. Ostil from my mommy
Kristine E. Ostil and daddy Jonathan;

4. I grew up with my mother until I was six years old, but my father provides for
my food, clothing, education and everything else I need;

5. From six years old to present, I am now staying with my grandmother

FLORENCE A. OSTIL and my aunt MELANIE O. PARONE as my mother was
already busy processing her papers to work abroad too;

6. I have witnessed how my Lola and Tita love and care for me while my Daddy
is in Aruba;

7. I learned that my daddy is processing my adoption for him to take me with

him in Aruba;

10. I hereby give my unqualified and unconditional consent to my adoption and I

am also very thankful for the love and care my mommy gave me; and

11. I am executing this Affidavit of Consent for the purpose of attesting to and
confirming the truthfulness of the foregoing facts and declarations, in
support of the Petition to be filed by my daddy.
