AFFIDAVIT OF LOSS

I, CHRISTIAN ANGELO FESTIN ABUEL, of legal age, Filipino citizen, with

residence and postal address at 12 Ranger St., Zone 4, South Signal Village, Taguig
City, after having duly sworn to in accordance with law do hereby depose and state:

That I am a holder and in possession of the ORIGINAL RECEIPT dated April 8,

2019, issued by Bayad Center located at 30-C Gov. Rodriguez Street, Signal Village,
Taguig City, covering the payment for Fibr Whole Home Wifi Plan 2099 (PLDT) with
Account No. 274009802;

That sometime on June 2019, while looking for the aforesaid Original Receipt in
my wallet where it was usually kept, I could no longer find the same, probably was
misplaced;

That I exerted all efforts to locate or find the said Original Receipt, but proved
futile and meaningless whatsoever;

That I am executing this AFFIDAVIT to attest to the truth and veracity of the
above statements for all LEGAL INTENTS and PURPOSES it may serve best.
