AFFIDAVIT OF CONSENT

I, DANTE BANAÑA DE VERA, of legal age, married, Filipino Citizen,

and with residence and postal address at Pucay Village, Marcos Highway, Baguio
City, after having been sworn to in accordance with law hereby depose and state
that:

1. I am the biological father of the minor ANGEL DANSSET

VENZON DE VERA, who was born on 07 September 2008;

2. Believing that it would be for my daughter’s own interest and benefit,

I hereby signify my written intent, freely and voluntarily to have my
daughter be adopted;

3. I also signify my written consent to have my daughter, ANGEL

DANSSET VENZON DE VERA, be legally adopted by Spouses Adam
Alexander Pearce and Maritess Deala Venzon, the latter being my
daughter’s biological mother, in accordance with law;

4. I am ready and willing, freely and voluntarily to abdicate my parental

authority over said child in favor of the above-named spouses, who are
both of legal ages, and possess the necessary civil and legal rights as well
as the financial capacity to raise, rear and support said child, upon
institution of the adoption proceedings and, thereafter, an order of
adoption be issued;

5. I execute this affidavit to declare our intention and consent to the

adoption of my daughter, ANGEL DANSSET VENZON DE VERA, as
well as to the truth of the foregoing facts;

IN WITNESS WHEREOF, I have hereunto set my hand this 7th day of

June 2016, at the City of Baguio, Philippines.
