AFFIDAVIT OF NO EMPLOYMENT

I, , of legal age, single, Filipino, and a resident of Bacolod, Lanao del

Norte, after being sworn to in accordance with law, do hereby depose and
state:

That as of present time, I am not employed in any private or

government corporation, company or to any individual person and hence,
I have no regular income;

That I have previously worked as a Waitress / Server in Panglao,

Bohol from 02 December 2013 until 17 December 2018;

That I have applied for the correction of my records specifically my

gender from MALE to FEMALE in the NSO Birth Certificate;

That I am executing this affidavit of unemployment to attest to the

truth of the veracity of the foregoing for whatever legal intents and
purposes it may serve best and to support my application for Correction
of Clerical Error for R.A. 10172 (GENDER) in my Certificate of Birth
before the Office of the Municipality of Bacolod, Lanao del Norte;
