Dear Member:

We are writing to inform you that Regence BlueShield and Harrison Hospital have been unable to reach
agreement on new contract rates which could impact the amount of money you have to pay to receive
services at Harrison Hospital and from its affiliated physicians.

Unless we can reach an agreement before then, our current contract ends on Aug. 1, 2014. We want you
to know how this change impacts your choice of medical providers and your out-of-pocket costs for the
care you receive.


Hospital Services:

After J uly 31, to be sure you receive your in-network level of benefits you should obtain services
at a participating hospital or urgent care center. A partial list of other hospitals and urgent care
centers in the area that are participating in the Regence BlueShield network are listed at the end
of this letter. You can also visit myRegence.com to find participating hospitals and urgent care
centers.

After J uly 31, if you receive care in the emergency room at Harrison Hospital, Regence
BlueShield will reimburse you directly for Regences allowed amount for covered services (less
any personal deductible, copayment or coinsurance amounts you may owe). You will be
responsible for making payment to the hospital for these services and may be subject to payment
for amounts up to the total billed charges. This difference could be substantial depending on your
benefits and the scope of services you receive.

If your benefit package does not include reimbursement for non-network hospitals and you
choose to receive elective, non-emergency services at Harrison Hospital after J uly 31, Regence
BlueShield will deny charges for those services. You may be financially responsible to the
hospital for the cost of the entire scope of services you receive.

If your benefit package does include reimbursement for non-network hospitals and you choose to
receive non-emergency services at Harrison Hospital after J uly 31, your claims will be processed
at the out-of-network benefit level, less any personal deductible, copayment or coinsurance
amounts you may owe. You will be paid directly for these services. You will be responsible for
making payment to the hospital for these services and may be subject to payment for amounts up
to the total billed charges. This difference could be substantial depending on your benefits and
the scope of services you receive.

or cancer, please call Regence BlueShield customer service at the number listed on the back of
your member ID card for assistance.


Professional Services:

Regence BlueShield continues to have a large network of contracted physicians and other
providers in Kitsap and Pierce counties, and surrounding areas. To ensure you receive your in-
network level of benefits, you should select a provider who participates in the Regence network.
You can visit myRegence.com to find participating network providers in your area.

If your benefit package does not include reimbursement for non-network providers and you
receive care from Harrison Health Partners providers after J uly 31, Regence BlueShield will deny
charges for those services. You may be financially responsible to the Harrison Health Partners for
the cost of the entire scope of services you receive from Harrison Health Partners providers.

If your benefit package does include reimbursement for non-network providers, and you choose
to receive care from Harrison Health Partners providers after J uly 31, your claims will be
processed at the out-of-network benefit level, less any personal deductible, copayment or
coinsurance amounts you may owe. Because the physicians will no longer be in the network, they
can bill you for any charges above our allowed amount, for which you will be responsible.

You will be paid directly for these services with a check jointly payable to you and the Harrison
Health Partners provider (co-payee check). The difference between what Harrison Health
Partners charges you for these services and Regence BlueShields direct payment to you will be
your personal financial responsibility. This difference could be substantial depending on your
benefits and the scope of services you receive.

If you are currently receiving ongoing care from a Harrison Health Partners provider for a
condition such as pregnancy or cancer, please call Regence BlueShield customer service at the
number listed on the back of your member ID card for assistance.

In the event that we are able to reach an agreement with Harrison Hospital on a new contract before
August 1, 2014, we will notify you. Otherwise, the transition will occur as outlined above. If you have any
