MOUSE ISSUE RESOLVED. PROVIDE RECEIPT FOR SERVICE.

MET WITH MANAGER AND DISCUSSED COMPLAINT. INSPECTED DOUGH FOR MOUSE FECES, DID NOT FIND
MOUSE FECES IN FOOD PRODUCT. INSPECTED HOLDING CABINETS FOR MOUSE FECES, DID NOT FIND MOUSE
FECES IN CABINETS. NO MOUSE FECES ON EQUIPMENT. MOUSE FECES ALL ALONG FLOOR WALL JUNCTURES
THROUGH OUT FACILITY.

MOUSE FECES FOUND ALONG THE WALLS THROUGH OUT THE FACILITY. SIGNIFICANT AMOUNT OF MOUSE
FECES. FACILITY IS CLOSED UNTIL FECES CLEANED AND ALL FOOD AND NON-FOOD CONTACT SURFACES ARE
CLEANED AND SANITIZED AFTER FECES REMOVAL.
Violations noted on this inspection report could result in license suspension and/or legal action. You are entitled to an administrative hearing on this
matter. A written request must be filed with the legal department, 3838 North Rural Street, Indianapolis, Indiana 46205, within 10 business days of the
issuance of this notice.
