Media Dept Camera Contract for home release

Students:

1. I understand that a camera will only be lent to me/my group on production of the correct
paperwork and contract signed by myself and a parent/ guardian if shooting off site. I
agree to keep the paperwork safe in my group project folder, and to ensure it is fully
completed before the equipment is released to me/ my group.

2. I understand that careless or silly behaviour can result in unnecessary damage being
caused to the cameras, and I promise to follow basic safety procedures as taught in class
when my group are responsible for the camera.

3. I agree to take the best possible care of the camera and accessories, and to report any
damage/ problems/ concerns to my teacher immediately.

4. I will not lend the camera to anybody else during my group’s booking unless authorised
by my teacher.

5. I promise to help check the equipment when I have completed my shooting session, and
before returning it to the Media Dept.

6. I understand the need for my group to return the equipment in full, at the time agreed on
our booking form, and I understand that we are all responsible for that.

7. I understand the seriousness of non or partial return of kit, and accept that I/my group
members will be sent to retrieve anything not returned immediately, including anything
left at home.

Please ensure that your son or daughter discusses the arrangements for each shooting
session with you so you are aware of their plans in advance. Please sign this contract and
help us reinforce to all our students the importance of health and safety, and personal
responsibility when borrowing equipment and shooting off-site.

1. You will ensure that the named student is able to travel to and from school safely when
in possession of the equipment they have borrowed

2. You have stressed to your child the need to take care, and to be responsible
when out on a location shoot, both in relation to personal and equipment safety

3. You have reinforced the importance of returning the correct kit to school on
time, as agreed in the contract

4. You will ensure the equipment is stored safely and securely whilst in your
possession

5. You reinforced that the equipment is not to be used by/ lent to anyone other than the
media student named on the booking form or named members of their group
