We, the official veterinary authority as concerned authority, confirm that the milk used in the production
of the goods being shipped under this L/C did not organize from Belgium and was free from dioxin and
any other contaminated matter as well as that the forage used for feeding the cows was not of Belgian
origin and was free from dioxin.

Ja, niżej podpisany Powiatowy Lekarz Weterynarii zaświadczam niniejszym, że mleko mleko
użyte do produkcji n/w towaru, wysyłane w ramach n/w akredytywy nie pochodzi z Belgii, nie
zawiera dioksyn i innych zanieczyszczeń w ilości przekraczającej obowiązujące normy w
mleku, a krowy nie były karmione paszą pochodzenia belgijskiego.
